Elmiron and Eye Damage

Elmiron received FDA approval in 1996 as a treatment for interstitial cystitis (IC), a bladder pain syndrome. After more than 20 years of use, however, researchers at Emory University in 2018 reported on six adult women patients with a unique type of degenerative visual disorder –- pigmentary maculopathy – that they linked to their patients’ chronic use of Elmiron.

The macula is the central area of the retina, the region of keenest vision. Pigmentary maculopathy is a form of macular degeneration that can result in irreversible retina damage, vision loss and blindness.

Symptoms include:

  • blurry vision,
  • night blindness,
  • difficulty reading,
  • loss of vision detail, and
  • dark spots on the retina.

Elmiron, manufactured by Janssen Pharmaceuticals, a division of Johnson & Johnson, is Pentosan Polysulfate Sodium (PPS), and was initially used for its anticoagulant properties in the treatment of varicose veins.

It was later found that the drug could provide clinical benefit for patients suffering IC, a condition that has been estimated to affect more than 1 million people in the United States, predominantly women. Elmiron remains the only FDA-approved drug for the management of IC.

Since 2018, there have been more than a dozen published reports and papers confirming the link between the long-term use of Elmiron and vision problems from maculopathy. These studies have shown that up to 25% of chronic users of Elmiron are afflicted with this condition.

Researchers suggest that long-term users of Elmiron who notice vision changes should undergo an eye examination with retinal imaging. The diagnosis of pigmentary maculopathy is usually made after imaging studies and a consultation with a retina specialist.
